Permission and Medical Release Form
Everyone attending Trek must fill out the Medical Release Form below. Youth attendees will need to have a parent sign the form. Adults may ignore the "permission" aspect of the form and sign the form for themselves. Please turn the signed forms into your ward leaders or a member of the stake YW or YM presidencies by April 30, 2015. Thank you!
